How to Request a Reissue

​If you have lost your Certificate, Competency Transcript or Statement of Attainment, you can request for it to be reissued by clicking below. Please note that a $99 reissue fee applies, including archive retrieval, staff time, printing and posting costs.
Replacement Certificates can take 5 - 15 working days, depending on record retrieval.​
