If you're under 18 and want to enrol in study with UNET, a parent or guardian needs to give their permission first. This short, two-page form is how they do it.
For students: Fill in your details (name, date of birth and the course you want to do) exactly as they appear on your enrolment form. Then give the form to your parent or guardian to finish and sign.
For parents and guardians: By completing and signing this form, you confirm that you:

- agree to our enrolment terms and conditions
- accept responsibility for course fees until your child turns 18, unless other arrangements have been made with your child's school. You can find fee details on the Diploma of Business page.
